For your convenience, the following forms are available to be downloaded from our site.  When you are ready to book your shipment, please complete the attached files and return them to us via fax (410 633 0556), email, or snail mail.

Shipping Information - This form provides us the specific information regarding your shipment.  Please provide as much information as possible.  Please note, the shipper is the person who legally owns the goods and is responsible for shipping them overseas.

Power of Attorney - This document must be completed and signed by the shipper.  This grants us authorization, from you to us, to arrange your shipment on your behalf.  This document does not have to be notarized.  This document ONLY authorizes us to manage your shipment.  It is called a limited standard power of attorney.

Terms and Conditions - This document is for your reference only.  There is nothing to complete.

Please rest assured, all of the information requested in this section will be treated as CONFIDENTIAL and will be used solely for the completion of the export documents required by U.S. Customs Service, the U.S. Department of Commerce, and the steamship line.  This information will NOT be released to anyone outside the above groups.
